| Hello guild member: On the next page you will fill out an entry form. After you finish the form you'll be presented a page to print that contains the information you entered. Print one copy to mail in with your check, and a second if you want a copy to keep for your records. You must be a guild member in good standing to submit an entry.
Entry forms must be received by March 15, 2010
